How We Collect And Use Your Personal information
What information do we collect?
A Better Ear (we) collects information that is required for your treatment and care. This will include your name, address, phone number and date of birth to make sure we can identify and contact you.
We also contact certain medical history and medication to help us diagnose and treat your hearing.
We may also ask for you Medicare card number or your pensioner card or DVA card number if it is required for us to manage your file with the Australian Governments Hearing Services Program.
If you provide your email address, we may use it to send appointment and review reminders and occasionally to tell you about special offers. We will never pass on or sell your email address to a third party.
How is your information collected?
Your information is collected in the following ways:
- Directly form you via our patient information and consent form or verbally during the consultation
- By telephone at the time of booking your appointment
- From letters or referrals for your doctor or other health professionals with your consent
Ensuring your information is correct and up to date
From time to time we may ask you to update your personal information to ensure we are keeping an accurate record of your health and have the most relevant and recent information to provide you the best care.
Accessing and correcting your information
A patient has the right to access their information and also to ask to correct details that they believe to be incorrect. This must be requested in writing to the Business Manager.
